The New Patient Paperwork is designed to inform us about the pain and discomfort you are feeling prior to your initial examination.  Please take time and be sure to indicate even the slightest concern you have.  Sometimes the severity of auto accident injuries can become exacerbated over time.  Also, share with us a little history and information about your overall health.

Along with the information about your health, there are also some required paperwork that will be shared with your insurance company and/or attorney.
